Our Project Sites

We collaborate with six clinics on the South Side of Chicago. Over the next five years, each clinic will implement all four branches of the project: patient education, provider training, clinic redesign, and community collaborations.

  • Access Booker Family Health Center ? Two cohorts of the patient Diabetes Empowerment Class have been held at this site, with very high rates of attendance. Provider Workshops were held jointly with ACCESS Booker Family Health Center and ACCESS Grand Boulevard Family Health and Specialty Center in March 2012 with good attendance and great discussion from the participants.
  • Access Grand Boulevard Health and Specialty Center ? The site staff are now leading their own patient Diabetes Empowerment Classes after three successful cohorts and are helping to create a Spanish curriculum. Provider Workshops were held jointly with ACCESS Booker Family Health Center and ACCESS Grand Boulevard Family Health and Specialty Center in March 2012 with good attendance and great discussion from the participants.
  • Chicago Family Health Center ? South Chicago ? A Spanish-language version of the patient Diabetes Empowerment Classes is in development in collaboration with this site and the Access Family Health Centers. Provider Workshops were held in 2011 for all clinic staff, including providers, medical assistants, and front desk staff.
  • Friend Family Health Center ? The patient Diabetes Empowerment Classes andÿProvider Workshopsÿare coming soon to this site.
  • The University of Chicago Kovler Diabetes Center ? The Kovler Diabetes Center and Primary Care Group are in their fourth cohort of combined patient Diabetes Empowerment Classes, and have completed the Provider Workshops with high levels of engagement and interactivity.
  • The University of Chicago Primary Care Group ? The Primary Care Group and Kovler Diabetes Centerÿare in their fourth cohort of combined patient Diabetes Empowerment Classes, and have completed the Provider Workshops with high levels of engagement and interactivity.
